Person-centered planning involves working together to create a Valued Life Plan, which is based upon the concepts of Giangreco, et. Al. (1993). This is a dynamic "living" plan that brings together a person's abilities with his/her dreams and desires. The planning process is delineated by five major life activity domains (Giangreco, et. Al., 1993), which include:
- A safe and stable home
- Being able to access a variety of places where meaningful activities occur
- Being part of a social network based upon personally meaningful relationships
- Having age appropriate personal choices and contacts
- Living in a safe and healthy manner
The object of this plan is to offer ideas to the consumer on how he/she may achieve his/her dreams and desires (valued life activities) while taking into consideration his/her abilities. Through a creative process of reviewing life long needs, the best map of the future can be made, including time lines for action.
